What to expect on your first visit with IBH

Please note, if you have not seen a practitioner in an IBH office location, you will be considered a new patient with IBH, regardless of whether you have seen one of the practitioners at a previous location (such as APS).

Also, we do not accept walk-in appointments, and do not provide crisis or emergency services. If you are in a current crisis or emergency situation, please visit the nearest emergency room (ER) or contact Crisis Services of North Alabama's suicide hotline at 1-800-691-8426 or (256) 716-1000. The suicide hotline is available 24 hours a day, 7 days a week.

After calling the office and providing us with the referral information, patients will be given the first available appointment. This will usually be within two to three weeks of the time the call is received. Please note that for your first visit, we will schedule you with a therapist to complete your intake. Every effort will be made to schedule the appointment at a time that is convenient for the patient.

Before arrival to the office, you will be asked to complete intake forms to provide us with some basic demographic information. Please make sure these forms are completely filled out before your scheduled arrival time. We will normally ask you to arrive 45 minutes prior to your initial appointment time. This is to ensure that we have all pertinent information in the system for the practitioner before you are seen. If you do not arrive early for your initial appointment, this may result in having to reschedule your appointment to a later date and being charged with a late fee of $100.

After these forms are completed, patients will be seen first by a licensed therapist, then by a nurse practitioner or psychiatrist. An initial assessment will be conducted to help identify and address your specific needs, with referrals made accordingly.

It is vital that our practitioners and clinicians receive accurate and complete information in order to ensure your treatment outcomes are maximized. Your first visit may take up to 2 hours. Our staff respects your time and schedule. We will make every effort to minimize wait time prior to your appointment.

Patient rights: Consumers of mental health services have the same general rights as other citizens of Alabama. However, IBH does maintain the right to terminate services for reasons of non-compliance or behaviors deemed disruptive to care at the practitioner's discretion.

Confidentiality: IBH considers all patient information and/or data to be confidential in nature cannot be not released or disclosed without the written consent of the patient. No patient or patient identifying information related to or regarding a patient can be divulged to any source, without a signed release of information by the patient, unless required by law.

Patient Responsibilities

IBH does not accept walk-in appointments and does not provide crisis or emergency services. If you have a crisis or emergency situation, please go to the nearest emergency room or call 1-800-691-8426 (Crisis Services of North Alabama suicide hotline).

If you do not show up for your first/initial appointment without notifying the office 24 hours in advance, you will not be rescheduled with a practitioner and your file will permanently be closed with all IBH offices. No exceptions will be made. A fee of $100 will also be charged.

After the initial intake, there will be a $100 charge per provider for missed appointments or late cancellations. Please notify the office 24-hours in advance if you need to reschedule or cancel your appointment. This fee must be paid in full before another appointment will be made/rescheduled.

If you arrive more than 15 minutes late for your appointment, your appointment will be rescheduled and a late fee of $100 will be charged.

After 2 missed appointments, your file is subject to be closed with this office permanently and you will not be rescheduled in any of IBH's offices.

If a practitioner deems your participation in treatment or care as dangerous, non-compliant, litigious, etc., the practitioner maintains the right to terminate services. Meaning, behavior directed towards staff or practitioners which could be considered as dangerous, violent, or offensive will not be tolerated and your chart may be permanently closed.

All copay's, deductibles and payment on account balances are due the date of you visit prior to seeing a Psychiatrist, Nurse Practitioner, or Therapist

It is you responsibility to ensure the office has accurate patient information and can reach you when necessary. Please notify office personnel of any changes in address, phone number, insurance, etc.

There is a $35 fee for all returned checks. If a check is returned for insufficient funds, IBH will no longer accept a check as payment from you.

Medical record fee: There will be a charge to copy/print records for your personal use and charges will apply accordingly.
